Responsible for assisting senior staff in guaranteeing the provision of psychiatric rehabilitative case management services to consumers residing in varied therapeutic supervised settings. Communicates and documents all observations and discussions utilizing established protocols. Addresses all treatment needs of consumer residents, assuring they reside in safe and therapeutic environments.
Assures the provision of psychiatric rehabilitative services to consumer residents, promoting Wellness and Recovery concepts for consumer residents residing in the least restrictive environment. Communicates observations and information about the consumer residents to the Director of Residential Services, Coordinator of Residential Services, RN, fellow co-workers, and the Treatment team utilizing established protocols. Ensures the safety of all consumer residents at all times, and immediately reports any facility management issues and concerns. Assists in the development of individualized treatment plans with consumer residents, and provides the identified intervention strategies in order to realize identified consumer driven goals. Documents all observations and discussions in shift to shift reports. Assists in the recreational, educational, and socialization activities for consumer residents. Engages residents in a racially, culturally, and gender sensitive manner. Adheres to established schedules, and participates in all scheduled trainings and staff development activities. Recognizes the signs of symptomatic or escalating problem behaviors, and utilizes crisis intervention strategies in order to deescalate potential crisis situations. Refers consumer residents to appropriate clinical personnel when needed. Recognizes signs of abuse, neglect and domestic violence and properly report incidents to authorities. Provides transportation for consumer residents to and from places of residence, appointments, treatment facilities, community agencies, schools, special outings and employment sites. Ensures the safety of all passengers at all times. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned.
Work requires the knowledge of theories, principles and concepts normally acquired through completion of Bachelor degree from an accredited college or university with a major in a Mental Health or other appropriate Human Services discipline OR licensed as RN in the State of NJ by the State Board of Nursing OR a combination of one or more years of college, plus related work or life experience together equaling four years OR the individual is required to have at least one year of related work experience if the four-year college education does not meet the prior requirements OR a License as a Practical Nurse plus two years of related experience; OR a High School diploma or the equivalent, plus four years of related work or life experience. Must have valid CPR and Basic First Aid certification. Valid driver�s license required, as well as compliance with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJHS Fleet Management & Safety Program. Bilingual (Spanish/English) preferred. Work requires the ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems.
