 Job Overview

 

Provide clinical case management including ongoing psychosocial assessment and treatment planning for patients within a variety of specialty areas in the hospital. Collaborates with multidisciplinary teams to provide appropriate discharge, ongoing and follow-up plans including the coordination of community and hospital resources and care.

Responsibilities

Develops and maintains an ongoing clinical caseload of patients within the specialty. Develops follow-up of needed interventions by social service identified by that division. Develops and completes a psychosocial assessment on all patients including treatment plans that are in the program and require social service analyses and intervention identified by the team. Provides patients with comprehensive clinical case management services including receipt of entitlements, advocacy, school referrals, legal system, hospital base and community resources as identified. Recognizes, and evaluates the level of severity and the need for immediate intervention especially in signs of abuse, neglect, violence for all ages and report to appropriate authorities as indicated according to policies. Provides appropriate crisis intervention for patients and family and staff either through own recognition or referral by other multidisciplinary teams. Participates and offers expert recommendations for plans during patient team meetings with follow-up for support and motivation of patients. Documents accurately and timely all interactions with patients, staff and agencies including care plan and follow-up strategies for patient non compliance. Communicates effectively with physicians, team members, patients and staff both inpatient and outpatient ensuring patient care is followed through by all members of the hospital team. Qualifications

Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Master's Degree in Social Work field. Licensure as a Social Worker by the State of New Jersey required.
