Job Overview

Responsible for assessing surveillance documents daily, identifying “high-risk” diagnosis/lab reports, examining corresponding current medical record for evidence of potential Hospital Acquired Infection, assembling data, and monitoring staff response as evidenced by initiation of appropriate isolation. Conducts targeted surveillance to determine outbreaks, reports communicable, infectious diseases to local Board of Health, MD, nursing/ambulatory sites in designated time frame, enters demographics into CDRSS, and manages patient placement according to protocol. Researches and develops education programs, following established hospital policy, regulatory guidelines and accrediting agency requirements. Ensures accurate and timely data entry to meet established deadlines for public reporting and possesses the ability to validate, analyze data and develop action plans; lead teams in the implementation of interventions as necessary. Works collaboratively with direct care providers to achieve implementation of best practice and patient safety. Responsible to provide services to all sites within the healthcare system, including Paterson Campus, Wayne Campus, St. Vincent's Nursing Home and SubAcute Rehab, Ambulatory Clinics and associated physician offices.

Responsibilities

Conducts targeted surveillance to determine outbreaks, reports communicable, infectious diseases to local Board of Health,  MD, nursing/ambulatory sites in designated time frame, enter demographics into CDRS, mange patient placement according to protocol.

Participates as a member of Infection Prevention and Control Committee, present data monthly/quarterly, provide analysis and recommend interventions as appropriate. Attend other committee/team, present Infection Prevention and Control reports in the same capacity as the. Infection Prevention and Control Committee

Researches, prepares, and presents educational programs for hospital staff and community that incorporate objectives relevant to Infection Prevention and Control policy and regulatory guidelines.

Collaboratively develops department/hospital policy /procedure based on standards as dictated by regulatory guidelines. Provide education and assess staff compliance through observation during rounds.

Acts as a resource for hospital, Employee Health Services and ambulatory sites by assessing issues and offering guidance for resolution based on hospital policy and acceptable Infection Prevention and Control  practice.

Access compliance with Environment of Care standards through the utilization of the  construction /renovation tool, daily rounds, weekly Administrative Rounds; recommends interventions as appropriate.

Monitors bulletins from local/State level; implement change as appropriate in support of the facility Disaster Preparedness plan.  Participates in Bioterrorism training courses that support facility process.

Develops patient and family education material that communicates healthcare system policy.

Meets all required competencies for department, unit and/or hospital.

Qualifications

Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Registered Nurse (RN), Diploma or Bachelor's degree and three years of work related experience. Licensure required as a Registered Nurse by the State of New Jersey, or able to acquire license within 60 days of hire. Public Health Nurse (PHN) :Bachelors degree. Licensed 3 years in public health or Medical Technologist (MT) Bachelors degree & member of American Society of Clinical Pathologists (ASCP) or equivalent, 3 yrs. lab experience or Bachelor of Science (BS) in health care related field, 3 years experience in allied health field. Attendance at an infection control course i.e. APIC training. Enrollment in Masters Program is preferred. Certification in Infection Control (CIC) is required after two years in the position and within 5 years of employment in the department. Recertification every 5 years after initial certification is required as per CBIC (Certification Board of Infection Prevention and Control). Proficient in word, excel, surveillance software program(s)
