Responsible for the admission and readmission processes and for the assessment of residents' response to treatment and care initiated by various members of the health care team and the accurate reporting of those responses. Monitor LPNs and CNAs performances to ensure completion of their assignments, including their documentation, in a safe and timely manner.
Admits residents timely, obtains orders, initiate care plans based on the needs and diagnosis. Administers direct resident care, based on nursing process and standards of care. Administers medications and treatments in an organized and timely manner to complete work in an eight hour shift. Obtains shift report and gives report to on coming shift on each resident from the care plan. Answers call lights. Communicates all pertinent information to the physician/APN, Nursing Manager, Nursing supervisor, and LPN for follow up using the SBAR process. Leads and assists the CNAs in the delivery of resident care. Provides daily assignment and updates assignments as necessary. Ensures CNAs documentation is completed timely and appropriately per policy and procedure. Assumes charge role in the absence of the Nurse Manager (NM). Provides input to NM regarding CNAs and LPN performance. Responds to emergency situations appropriately. Identifies problems related to diagnosis. Addresses them, initiate/revise care plans to reflect problems, goals and interventions. Revises nursing care plan accordingly to resident's progress. Maintains the security and confidentiality of the resident's medical record. Participates in the Interdisciplinary conferences prepared to discuss resident's progress toward meeting goals and methods to improve or enhance quality of life. Assumes the role of a multidisciplinary team member to assure quality of life. Attends and participates in medical decision conferences and continuing education programs as needed. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned. Special Projects
Work requires the knowledge of theories, principles, and concepts normally acquired through completion of two to three years of occupationally specific education beyond high school or an Associate's Degree in Nursing and previous experience in long term care preferred. Licensure required as a Registered Nurse by the State of New Jersey.
