Counseling and case management services for individuals within the department of Behavioral Health. Counseling techniques are utilized to engage clients in treatment. Assessment and treatment planning are utilized to help clients reach personal goals. Clients are assisted in obtaining entitlements and linkages to other community resources/services. Outreach activities to clients who are ambivalent about services or who miss appointments.
Counsels clients using a variety of therapeutic approaches. Completes psychosocial assessments and develops individualized rehabilitation/service plans with clients, incorporating client preferences in the process. Demonstrates knowledge and ability in providing comprehensive services, such as entitlements, advocacy and referrals to community resources. Demonstrates the ability to engage, motivate and treat clients effectively and in a culturally competent manner, as demonstrated by review of cases and face to face contacts. Identifies and responds appropriately to crisis situations that may arise in the office and in the community with clients and/or family members/significant others. Conducts outreach activities to clients who may be ambivalent about services or who miss appointments, in an effort to link them to appropriate community services. Consults with supervisor and multi-disciplinary team for problem solving and guidance in difficult cases, e.g. regularly scheduled supervision meetings. Completes all documentation in a timely, complete and accurate manner, in accordance with all accrediting bodies. Collaborates with other programs/agencies on shared cases when necessary. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned.
Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in Social Work, Psychology, Psychiatric Rehabilitation or mental health related discipline. Bilingual (Spanish/English) preferred. Licensure preferred as a Certified Social Worker (CSW) by the State of New Jersey. Work requires the ability to counsel and educate consumers and their families and to resolve problems that require the use of basic mental health knowledge. Valid driver’s license required and comply with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJHS Fleet Management & Safety Program.
