Produces data reports and completes data analyses for value-based hospital programs including DSRIP (Delivery System Reform Incentive Payment Program), Bundled Payment Programs, Comprehensive Care for Joint Replacement, and CHF/COPD (Congestive Heart Failure/Chronic Obstructive Pulmonary Disease) Readmission Programs. Coordinates monthly/quarterly action planning and goal setting for the value based programs which will assist in increasing reimbursement rates and decrease CMS (Centers for Medicare and Medicaid Services) penalties for the healthcare system. Works with IT to create performance dashboards. Supports value-based purchasing initiatives and oversees value based purchasing meetings. Works with staff/physicians/leadership to ensure that accountability structures are in place for strategy elements and outcome goals. Tracks hospital performance on specified measures and compares the data to national benchmarks or other comparative data. Works collaboratively with the department/hospital staff and leadership, as needed, to identify and facilitate initiatives that will continuously improve value based outcome measures. Performs department administrative duties, as requested.
