Evaluates, plans, directs and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability. Writes evaluations, progress notes/reports, discharge summaries and maintains patient files. Counsels patients and/or family members regarding Rehabilitation diagnosis, plan of care and prognosis. Maintains current knowledge of regulatory/legislative policies as they apply to the profession of Physical Therapy and adheres to same.
*FIM (Functional Independence Measurement)certification to be taken following employment and training at Wayne, then certification must be taken through the rehab program (Uniform Data System for Medical Rehab-UDSmr).
*For EIP (Early Intervention Program employees only: Valid driver’s license required and ability to comply with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJH Fleet Management & Safety Program.
