Facilitates processes to expedite patient throughput, coordinates prioritizes and optimizes patient flow activities. Assist in the coordination and prioritizing of Inpatient bed assignments including transfers from external hospitals. Facilitates the acceptance of external transfers and serves as the liaison between physicians, hospitals, and other care providers who seek to refer to SJHMC. Provides clinical expertise, quality assessment and medical communication for a safe appropriate transfer. Evaluates processes tracks and collects data on all areas within the Logistics realm. Coordinates and refines a customer centered process for the Bed management, Transfer Center and Transport Departments. Supports /guides the Patient Transport Dispatcher in prioritizing patient transport assignments. Acts as a liaison for the patient transport dispatcher in times of conflict.
Coordinates all hospital to hospital transfers utilizing the Call Copy and Transfer Center applications. Communicates with all necessary professionals to facilitate the transfer. Obtains and enters accurate vital information electronically into the data base. Performs follow up calls to assure the patient transfer is proceeding as planned. Reports issues or concerns to Manager or Director in real time when necessary and communicates all others either verbally or via email in a timely fashion. Provides the overall direction, guidance and coordination to health care providers in the achievement of patient throughput utilizing infection control principles and medical necessity criteria. Serves as the clinical resource person for the Bed Coordinators. Ensures that patients move efficiently through the system. Collaborates with MD, Care Managers, Unit Managers ,Intensives and transport to ensure timely coordination of transfers and discharges both internal and external . Serves as the ambassador to support department and hospital goals. Communicates High Census alert to appropriate levels. Attends daily bed Huddles and communicates hospital wide requirements to meet the needs of our patients both present and anticipated arrivals. Works with the Bed Huddle Committee to create a plan for improved throughput . Rounds on all clinical areas to determine bed availability. Ensures that Bed availability is communicated via the electronic bed board. Assists/guides primary nurse with issues impeding flow. Performs follow up calls on in house transfer delays. Reports extended delays to Unit Managers. Monitors the Bed management System, works to ensure that EVS and Transport resources are deployed timely. Trouble shoots problems with the electronic system and communicates problems to Tech Support, ITR, Telecom and Logistics Management in a real time when feasible. Participates in policy development, education and training of new employees. Serves as resource person on the weekends and off shifts to all users of the CMS system. Collects data from all logistics applications when requested. Identifies areas of concern and reports to the Manager/Director. Assists in setting transfer protocols and utilizing such to facilitate the rapid acceptance of acutely ill transfers. Differentiates by diagnosis and contacts appropriate specialty MD. Maintains on call schedule for the Transfer Center department. Serves on Committees as assigned. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned. Special Projects
Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in Nursing or closely related field and five to seven years of previous work related experience in Acute Care Hospital, preferably in the Critical care area, with diverse clinical background. Care Management experience is beneficial. Successful completion of BLS required; ACLS preferred. Ability to work independently, set priorities, accept and adapt to change, must be able to multi task and posses excellent customer service skills, computer skills and negotiating skills. Licensure required as a Registered Nurse by the State of New Jersey.
