     Job Overview


    Responsible for providing ongoing psychosocial assessment, treatment and discharge planning to a variety of patients within the medical setting. Provides clear communication across the continuum to improve patient outcomes. Responsible for an assigned caseload of patients.


    Facilitates and communicates timely and appropriate discharge plans to all patients while coordinating with external facilities, regulatory agencies and community programs. Consults with supervisor/director and multidisciplinary team for problem solving and guidance in difficult cases as needed. Demonstrates knowledge and ability in providing comprehensive service delivery such as entitlements, advocacy and referrals to community resources. Documents in a timely manner all contacts, interventions and outcomes in patients charts and electronic records. Screens/interviews all "high risk" admissions or referrals. Completes psychosocial assessments and screenings on all patients as appropriate. Actively participates in departmental performance improvement, special projects as assigned and coverage of absent workers as required. Attends relevant seminars and conferences in order to meet state licensure requirements and maintain knowledge of current developments in the profession. Provides crisis intervention services to patients and families. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned.     Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Master's Degree in Social Work field. Licensure as a Social Worker by the State of New Jersey required.
