Under the direction of the RN/LPN assists with basic nursing care and activities of daily living on the nursing unit and within the facility. Documents in all logs accurately and timely. Responsible for assisting in maintaining or improving the quality of life for all residents.
Makes rounds and communicates with off-going and on-coming shift to assure that each resident is safe and comfortable at the change of shift. Accountability Rounds form is signed by off-going and on-coming certified nursing assistant. Completes assignments in a timely manner. Prioritizes work based on resident needs. Promotes residents abilities rather than disabilities. Ensure all residents within the facility have legible ID bands in place, and those residents at risk for falls, have appropriate bands. Ensure those residents at risk for falls, have all fall prevention items in place. Assists residents with meals and snacks; assists with feeding and reports food and fluid intake to RN?LPN. Also, reports Bowel and Bladder pattern to RN/LPN. Assists RN/LPN in implementing the residents care. Contributes information on a day to day basis regarding resident status. Maintains confidentiality. Treats residents with respect, reports all resident concerns, accidents and incidents to nurse/manager/supervisor. Ensures abuse/injury reporting in timely manner. Documents ADLs, and in all assigned logs, accurately and timely. Answers residents call light within 2 minutes, responding to residents need or providing appropriate assistance. Maintains necessary and required in-service hours in keeping with regulations. Accompany residents to outside appointments as necessary. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned. Special Projects
High School diploma or equivalent preferred and one to two years of previous work related experience. Certification by the State of New Jersey as a Certified Nursing Assistant required. Minimum of one year experience in a long-term care setting preferred.
