Assists patients with activities of daily living, provides basic care to acutely ill patients with emphasis on returning patient to former functional status, and assists in maintenance of a safe and clean environment under the supervision of the Registered Nurse.
Assists clinicians with patients in activities of daily living such as basic personal hygiene, skin care, range of motion exercise and/or feeding; take weight/height measurement; records result, provides general orientation to room. Documents according to policy. Answers call lights in a timely carative manner and tends to the immediate needs of the patient. Informs nurse(s) responsible for specific patient requests or complaints, as well as changes in pt's. condition. Monitors vital signs, offers and empties bedpan/urinals as well as ambulates patient to and from bathroom. Collects specimens. Provides post mortem care. Carries out selected treatments as directed by the RN. Orders, restocks and maintains supplies for the patient care. Responsible to pick up STAT request (requisitions), delivers to appropriate area and return items to unit. Performs messenger duties to the Lab, Pharmacy and Central etc. Maintains patient daily hygiene, provides AM and PM care and additional care as needed. Reports any observations or changes in skin condition to the Nurse responsible for the patient in a timely manner. In collaboration with Interdisciplinary Team, adapts care to encourage patient independence so that functional ability is maximized and patient maintains as much independence as possible. Delivers care in accordance with National Patient Safety Goals; makes pertinent clinical observations when providing care and reports findings to Interdisciplinary Team. Ensures that the patient environment is correctly maintained and provides care in accordance with safety regulations and current infection control practices. Provides patient transportation to and from testing treatment areas, transport patients personal belongings to appropriate areas, transport deceased patients to the morgue, assist patient in and out of bed, assist patients onto stretchers and in and out of wheelchair. Assists patients with menu selection, provides food service delivery to patients in accordance with diet order; checks accuracy of food delivered against diet order; assists patients in opening food containers. Observes, calculates, and records intake of food and beverage consumption. Provides patients with menus. Collects complete menus and deliver to the Nutrition Office. Provides patients with missing food items, late trays or patient requests congruent with the diet order. Assists and feeds patients as directed. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned.
Work requires a High School diploma or equivalent and less than one year of previous work experience as a Nursing Assistant is preferred. Ability to identify and report clinical and environmental observations to the Nurse.
