Responsible for utilizing the Nursing Process to an age specific group of patients and their significant others using existing resources including time, material and personnel. Communicates and collaborates effectively with patients/significant others and all members of the health team. Documents nursing activities and patient outcomes. Responsibilities include but are not limited to medication administration, physical assessment, phlebotomy, point of care testing and clinical patient education.
Implements and performs age specific assessment and appropriate nursing care interventions according to prescribed therapeutic regimen and reports abnormalities to MD/APN promptly with accurate documentation. Plans and implements specific nursing interventions with each patient and family to coordinate the nursing/medical plan of care for optimum patient outcomes. Evaluates patient outcomes to determine the patient/significant other response to the plan and interventions, involving the patient/significant other when appropriate. Revises the plan and intervention as needed in a timely manner. Accesses continuing education opportunities appropriate to the area of discipline and responsibilities. Participates in the in-service education sessions provided by the institution and outside agencies. Administers medications and assist the patient with treatments, procedures and referrals following all applicable medication policies and procedures. Performs all required testing on patient with accuracy and documents results in a clear concise legible manner. Complies with Infection Control policies and procedures Maintains a knowledge base and performs age specific patient assessment recognizing and analyzing physical, psychological and spiritual human responses to illness and/or life processes. Tracks patients/significant others the necessary and appropriate concepts to effect self-care and/or optimum level of functioning. Meets the individualized educational needs of patients/significant others by utilizing available material and personnel resources, clinical references, disease process, growth and development. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned. Special Projects
Knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in Nursing or closely related field preferred. Licensure required as a Registered Nurse by the State of New Jersey.
