     Job Overview


    Serves as the point of contact for referring physicians, patients/families and caregivers to provide resources and assistance with accessing clinical, supportive care and financial services offered and available In the St. Joseph Healthcare System and community. Coordinates activities of the multidisciplinary team to ensure there is coordination of services and continuity of care for the patient. Anticipates barriers to care and services and intervenes to ensure the continuity of efficient/timely care. Facilitates transitions of care to assure that patients have appropriate referrals and follow-up appointments post discharge. Delivers and coordinates patient education to ensure quality outcomes including the patient's compliance with post discharge plan of care.


    Follow all inpatients with a specific primary diagnosis (ex. heart failure) throughout their hospital stay and transition period until they are seen by a health care provider in an outpatient setting. Collaborate with care managers. Facilitate patient appointments including those for laboratory tests, other diagnostic tests, specialty physicians, social workers, dieticians etc. Assist patients and families in understanding their diagnosis, treatment options and resources available. Develop and provide patient and family education and counseling on lifestyle changes necessary to maintain optimal health while living with heart failure. Provide patient/family with tools to manage their care. Ensure patients have access to care, are compliant with follow-up appointments and adhere to professional recommendations for their care. Data collection, outcomes measurement, analysis of data. Develop a scorecard to compare data with benchmark data and report results on a monthly/quarterly basis to Quality/Process Improvement Department. Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, social services staff, etc. Employs strategies to promote health and a safe environment: Provides individualized health teaching and promotion. Demonstrates competent behavior in the professional role, including activities related to quality of practice, education, professional practice evaluation, collegiality, collaboration, ethics, research, resource utilization, and leadership. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned. Special Projects Practices in accordance with established regulatory standards.


    Knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in Nursing or closely related field preferred and three to five years of previous work related experience.. Licensure required as a Registered Nurse by the State of New Jersey. Successful completion of BLS, a department specific orientation and department specific certifications such as PALS, ACLS, etc.
