Job Overview

Responsible for counseling and case management services for individuals within the department of Behavioral Health. Counseling techniques are utilized to engage clients in treatment. Assessment and treatment planning are utilized to help clients reach personal goals. Clients are assisted in obtaining entitlements and linkages to other community resources/services. Outreach activities to clients who are ambivalent about services or who miss appointments.

Responsibilities

  • Develops and implements treatment strategies to engage, support and stabilize clients in a culturally competent manner, as evidenced by spending a minimum of 65% of time completing face to face with clients.
  • Conducts various therapy modalities (individual, group, family) in an effective manner, where 10% of patients graduate as a result of achieving treatment goals.
  • Completes comprehensive intake assessments and interprets data to formulate a DSM IV TR diagnosis and ensure compliance with intake assessment policy, as evidenced by review of 2 new intakes per quarter during the medical record review process.
  • Uses evidence-based models of treatment appropriately and effectively, as evidenced by review of 5 treatment plans per quarter.
  • Participates in multi-disciplinary team meetings or weekly clinical supervision, as a way to ensure optimal quality care.
  • Completes USTFs, encounter forms and other required documentation, in compliance with policies and procedures.
  • Documents services provided to clients in a timely, legible, accurate and comprehensive manner and maintains medical records, in compliance with policies and procedures.
  • Provides crisis intervention/stabilization in a safe and culturally competent manner, in a way to de-escalate the crisis and maintain safety for the client and staff.
  • Provides case management services, including advocacy, linkage and referrals, collaboration with other agencies and other coordination of care services.
  • Meets all required competencies for department, unit and/or hospital.
  • Responsible for performing all other duties as assigned.

Qualifications

Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Master’s degree in social work, psychology or closely related field, Bi-lingual (Spanish/English) preferred. Licensure required commensurate to degree as required by the State of New Jersey. Work requires the ability to prepare detailed assessments of patient referrals, counseling individual, group or family members, or perform other duties requiring a comparable level of communication skill. Proficiency of Microsoft Office.
