 Job Overview

 

Evaluates, plans, directs and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability. Writes evaluations, progress notes/reports, discharge summaries and maintains patient files. Counsels patients and/or family members regarding Rehabilitation diagnosis, plan of care and prognosis. Maintains current knowledge of regulatory/legislative policies as they apply to the profession of Physical Therapy and adheres to same.

Qualifications

Graduate of Accredited Physical Therapy Program, Current NJ State License in Physical Therapy. BLS (Basic Life Support) certification required at time of hire or within first 3 months of hire. FIM (Functional Independence Measure) certification to be taken following employment and training at Wayne, then certification must be taken through the rehab program (Uniform Data System for Medical Rehab-UDSmr). For EIP (Early Intervention Program) only: Valid driver’s license required and comply with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJH Fleet Management & Safety Program.
