 Job Overview

 

Provides mental health screening assessments and makes appropriate interventions and referrals, in consultation with Supervisor and the on-site or on-call psychiatrist. Provides a full range of mental health treatment services 24/7, including: Mobile Outreach/Screening, responding to hotline calls, assessments, crisis counseling, case management and participates in interdisciplinary treatment planning, record keeping, consultation and education. Provides care to patients ranging in age from child through geriatric. Acts as clinical liaison as necessary between psychiatrists, RN and ER attending.

Responsibilities

Performs comprehensive clinical assessments and interpret data to formulate a DSM IV TR diagnosis. Collaborates in the formulation of treatment/service/discharge plans with a multi-disciplinary team. Recognizes, evaluates, gather collateral information and develop interventions to address risk factors related to potential for violence and dangerousness to self or others. . Provides crisis intervention/stabilization and aggressive behavior management to consumers in a safe, culturally sensitive and gender specific manner for patients in our care, both on-site and off-site. Provides clinical case management services, including but not limited to: assessment, referral and linkage to medical and community services, follow-up, outreach and coordination of service delivery. Documents services provided to consumers and maintain medical records in accordance with established governing standards in a timely, legible, accurate and complete manner. This includes, completion of log book entries, USTF's, and patient follow-up. Provides hotline coverage and cover telemedicine services. Conducts mobile outreach and screening of consumers who may be in need of commitment, in consultation with the on-call/on-site psychiatrist and Shift Supervisor Provides consultations, training and technical assistance to other community providers, municipalities and family members. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned. Special Projects

Qualifications

Work requires the knowledge of theories, principles and concepts normally acquired through completion of a Master’s degree in social work, psychology or closely related field; one year of experience in a psychiatric setting plus three to six months of on-the-job training and orientation or Bachelor's degree and two to three years of previous experience. Valid driver’s license required, as well as compliance with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJHS Fleet Management & Safety Program. Work requires the analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems. Work requires the ability to prepare detailed assessments of patient referrals, counseling individual, group or family members, or perform other duties requiring a comparable level of communication skill. Work requires providing direct, age specific patient care as appropriate to treatment setting.
